  1. Online EducationCorps Application 
  2. SDUSD Volunteer Application, Background Check, and Code of Conduct
  3. CANRA Training Upon completion email us a copy of your certificate
    • Below we have provided the information need to access the forms:
      • Registration code: ucsdedu
      • Organization:  University of California San Diego – Center for Student Involvement Community Service
      • Complete the following modules: Abuse Risk Management for Volunteers + CANRA Mandated Reporter
  4. A scanned copy of a government issued ID
  5. scanned copy of TB Test (taken within the last two years)
